What is a SLAP tear of the shoulder?

The shoulder is a ball and socket where the arm bone (humerus) is kept in the shoulder socket by a network of four muscles which help move and stabilize the shoulder joint. A SLAP tear is an injury to the labrum of the shoulder. The labrun is the ring of cartilage that surrounds the socket of the shoulder joint.  SLAP stands for Superior Labral Tear Anterior and Posterior.

In a SLAP injury, the top (superior) part of the labrum is injured, and the tear occurs both in front (anterior) and back (posterior) of where the biceps tendon attaches to the labrum.  A SLAP tear can be caused by a motor vehicle accident, a fall onto an out-stretched arm, or rapid or forceful movement of the arm.

Symptoms of a SLAP tear

Symptoms of a SLAP tear may include:

  • Popping, clicking or catching in the shoulder.
  • Pain when you move your arm over your head.
  • A feeling of weakness or instability in the shoulder.
  • A feeling like the shoulder is going to “pop out of joint.”
  • Aching pain.

Diagnosing a SLAP tear of the shoulder

Your doctor will perform a physical examination to determine if you have any signs or symptoms that are consistent with a SLAP tear. Then, if the doctor suspects a labral tear, an MRI will typically be ordered.  A dye is injected into the shoulder to further define the soft tissues of the shoulder and identify any tears. However, our injury lawyers have worked on many shoulder accident cases where the MRI does not identify a SLAP tear, so your doctor may recommend further exploration by arthroscopy.

In many cases, the initial treatment for a SLAP shoulder injury is use of pain and anti-inflammatory medications to help reduce pain and swelling until the injury heals on its own. Physical therapy is also recommended to help restore movement and strength in the shoulder.  However, if your symptoms do not improve with these measures, your doctor may recommend arthroscopic surgery.  During arthroscopy, small incisions (approximately 1 cm in length) are made in the shoulder, through which a fiber optic camera and other instruments are inserted to perform the surgery.  The shoulder is visually examined, including the cartilage, tendons and bursa of the shoulder, and any necessary procedures, including repair of the SLAP tear, are then performed.
